The Washington Attorney General's Office, through its Medicaid Fraud and Abuse Division, led the investigation.

The settlements represent part of a broader enforcement effort by the state's MFA division, which has recovered approximately $81 million for the Medicaid program through civil prosecutions. Attorney General Nick Brown emphasized that "accuracy and honesty from healthcare providers are fundamental to the integrity of the Medicaid system."

Dental practices and other providers should audit billing practices for systematic coding errors, ensure documentation supports all submitted claims, and verify medical necessity before billing.
